When the part gets wider

Thinning Hair

A part that keeps getting wider, a ponytail that feels thinner, more scalp showing under bright light: for most people, this is gradual, inherited thinning, the slow shrinking of hair follicles over years. It is common and treatable, but the first job is to confirm that is what it is, because sudden or patchy loss is a different story. At Santa Barbara Face & Body, Dr. Allison Pontius sorts out the cause before recommending treatment.

AT A GLANCE

Thinning hair is most often androgenetic alopecia, a gradual, patterned thinning driven by a genetic sensitivity of the follicles to hormones. At Santa Barbara Face & Body in Santa Barbara, California, a board-certified facial plastic surgeon confirms the cause, then treats it, which can include biologic hair regeneration with PRF alongside the medical options with the strongest evidence.

Most thinning hair is androgenetic alopecia, pattern thinning that runs in families. The hair follicles are sensitive to a hormone, dihydrotestosterone, and gradually shrink, so hairs grow finer and shorter over years. It progresses slowly and is treatable. Sudden shedding or patchy bald spots are different causes and should be evaluated before any cosmetic treatment.

What it is

Thinning hair usually means one of a few things, and telling them apart is the first step.

  • Pattern thinning (androgenetic alopecia). The most common cause, gradual and inherited. In women it shows as widening of the part and thinning at the crown, in men as a receding hairline and thinning at the top.
  • Sudden shedding. A temporary, diffuse shed months after a stressor like illness, surgery, or childbirth, which often recovers on its own.
  • Patchy loss. Round bald patches or scarring, which are separate conditions that need medical evaluation.

This page is about the first kind, gradual pattern thinning, which is the most common and the one cosmetic treatment addresses. The other two are flagged so they get the right care instead.

What causes it

Pattern thinning is genetic and hormonal. The follicles carry an inherited sensitivity to dihydrotestosterone, a hormone derived from testosterone, and under its influence they gradually miniaturize, producing finer, shorter hairs each cycle until they stop. It runs in families, affects both men and women, and tends to progress slowly over years.

Because it is driven by an ongoing hormonal process, treatment works by slowing that process and supporting the follicles, and results are maintained rather than permanent, which is part of setting honest expectations. Sudden or patchy loss, by contrast, has different causes worth identifying first.

What kind of thinning is it

This is what the first visit confirms, because the cause decides the plan.

What you noticeUsually meansThe route that fits
A gradually widening part or thinning crownPattern thinningConfirm, then treat, which can include biologic hair regeneration (PRF)
A receding hairline and thinning on topMale pattern thinningMedical options plus in-office treatment
A heavy, diffuse shed after a stressorTemporary sheddingOften recovers; supportive care and time
Round bald patches or scarringA separate conditionMedical evaluation before any cosmetic treatment
Thinning with other symptomsPossible medical causeWorkup first, including bloodwork if indicated

The value is not the list. It is that a physician confirms this is pattern thinning, and rules out a medical cause, before treating, so the plan fits the actual diagnosis rather than assuming.

Why Santa Barbara Face & Body

Hair loss is a field full of overpromises, so the useful thing a physician brings is honesty: confirming the cause, being clear about what each option can and cannot do, and combining treatments sensibly. Dr. Pontius evaluates the thinning first, offers biologic hair regeneration with PRF as one in-office option, and is straightforward that the medical treatments with the strongest evidence, and realistic, maintained results, are part of the conversation. A physician reading the scalp is what keeps expectations honest.

An in-office option PRF uses your own platelets, in a fibrin matrix, to support the follicles

When it is worth a consultation

Worth coming in if you

  • See your part widening or your ponytail feeling thinner over time
  • Notice a receding hairline or thinning at the crown
  • Want the cause confirmed before spending on treatments
  • Want an honest read on what actually helps

Better to talk it through first if you

  • Have sudden, heavy shedding or patchy bald spots, which need evaluation
  • Are pregnant or breastfeeding, since some hair medications are avoided
  • Have thinning alongside other symptoms, which may point to a medical cause

What to expect from a visit

Dr. Pontius examines the pattern and density of your thinning, asks about family history, timing, and health, and orders bloodwork if a medical cause is possible. If it is pattern thinning, the plan is laid out honestly, which can include biologic hair regeneration with PRF and the medical options with the best evidence, with realistic expectations about maintained results. If it looks like a different cause, that gets the right care first.

Is thinning hair reversible?

Pattern thinning can often be slowed and partially improved, but it is an ongoing genetic and hormonal process, so results are maintained with continued treatment rather than cured once. The earlier it is addressed, while follicles are still active, the more there is to work with.

Do you use PRP or PRF for hair?

At Santa Barbara Face & Body, Dr. Pontius uses platelet-rich fibrin (PRF) rather than conventional PRP. PRF is prepared from your own blood without additives, forming a fibrin matrix that releases growth factors gradually; it is applied to the scalp, often with SkinPen® Elite scalp microneedling, as one part of a plan alongside the medical options with the strongest evidence, and is offered with honest expectations about what it can and cannot do.

What are the medical options?

The medicines with the strongest evidence are topical minoxidil, available over the counter, and oral finasteride, a prescription for men, both FDA-approved for pattern hair loss. They are often the foundation, with in-office treatment like PRF added. A physician will tailor this to you and to whether you are a candidate.

My hair is suddenly shedding a lot. Is that the same thing?

Probably not. A sudden, heavy, diffuse shed, often months after illness, surgery, or childbirth, is usually a temporary type that recovers on its own, and patchy bald spots are a separate condition. Both are worth evaluating rather than treating as pattern thinning.

Will treatment regrow all my hair?

Realistically, the goals are to slow the thinning, thicken existing hairs, and support some regrowth, not to restore a full head of hair. A physician who is honest about that, and about maintenance, is more useful than a promise of complete regrowth.
