Understanding a hollow, tired look

Volume Loss & Hollow Cheeks

If your face looks flatter or more drawn than it used to, and skincare has stopped making a difference, the reason is usually underneath the skin rather than on it. The support that once rounded your cheeks and temples thins with time. At Santa Barbara Face & Body, Dr. Allison Pontius finds where that volume went before deciding how to replace it.

AT A GLANCE

Facial volume loss is the thinning of fat, bone, and collagen beneath the skin that leaves cheeks and temples looking hollow or flat. At Santa Barbara Face & Body in Santa Barbara, California, a board-certified facial plastic surgeon assesses which layer has been lost and restores it with filler, collagen-building injections, or fat transfer.

A hollow or flat face reflects loss of support underneath the skin. Fat pads shrink and slide, facial bone remodels, and collagen declines, so the surface has less to rest on. Restoring the look means replacing what was lost in the layer it was lost from.

What it is

Faces are built in layers: skin on top, then fat, then muscle and bone. The rounded look of a younger face comes mostly from the fat pads and the bone beneath them, not from the skin itself. When those deep layers thin, the skin has less to drape over, so cheeks flatten, temples sink, and the folds around the mouth deepen into shadows.

This is why a good moisturizer or a skin-tightening device can only do so much. They work on the surface, and the change is one or two layers down.

3 layers volume is lost in fat, bone, and collagen, not just in the skin

What causes it

  • Fat pads shrink and shift. The compartments that pad the midface lose fat and slide downward with age, which hollows the upper cheek and heavies the lower face.
  • Bone remodels. The facial skeleton loses projection over time, so there is less scaffold under the soft tissue.
  • Collagen declines. The firm, springy support in the skin and deeper tissue thins with age, and sun exposure speeds it up.

A faster version can follow rapid weight loss from a GLP-1 medication, because fat leaves the face along with the body. That has its own path, covered on the Ozempic face page.

Where the volume went, and what replaces it

The useful question is not “which filler,” it is “which layer.” That is what a physician read answers, and it points to different tools.

What you noticeUsually meansThe route that fits
Flat upper cheek, a tired midfaceDeep fat pad and bone lossA hyaluronic acid filler or a collagen-building biostimulator, placed on the structure
Broad, diffuse thinning across the faceWidespread collagen and fat lossCollagen-building injections over several sessions
A wish for a lasting, one-time restorationA candidate for surgeryMoving your own fat into the face
Hollowing after fast weight lossGLP-1 related volume lossAssessed on the Ozempic face path

The point is not the menu. It is that Dr. Pontius reads which layer is empty, and for surgical fat restoration works alongside Dr. Alain Polynice, so the plan follows your anatomy rather than a single product.

Why Santa Barbara Face & Body

Most practices that treat hollowing offer one tool and fit every face to it. Here all three real routes sit under one roof: precise hyaluronic acid filler, collagen-building biostimulators for diffuse loss, and surgical fat transfer for a lasting restoration. That range is what keeps the recommendation honest, because the plan can match your face instead of the practice’s inventory, and the physician reading you is the one who treats you.

When it is worth a consultation

Worth coming in if you

  • See flattening in the cheeks or hollows at the temples
  • Notice folds around the mouth reading as shadows, not just lines
  • Have lost facial fullness after weight change
  • Feel you look tired or drawn no matter how you rest

Better to talk it through first if you

  • Are pregnant or breastfeeding
  • Are early in a weight loss course and still changing, since timing matters
  • Are unsure whether the issue is lost volume, a line, or loose skin, which the consultation sorts out

What to expect from a visit

Dr. Pontius looks at your face in motion and at rest, in good light, and maps which areas have lost support. You leave understanding which layer is involved and the realistic order of restoring it, whether that is a single treatment or a staged plan over time.

What we offer for this

People come in about hollowing and leave with a plan built from a few routes. Which one fits depends on where the volume was lost.

Why do my cheeks look hollow when my weight is stable?

Because the change is under the skin, not on the scale. The fat pads that pad the midface shrink and drift down with age, and the bone behind them loses projection, so the cheek flattens even when your body weight has not changed.

Will filler make me look overdone?

Not when it is placed on the structure that was actually lost, in measured amounts. Overfilling happens when volume is chased at the surface instead of restored in the deep layer. The goal here is to rebuild support so the face reads rested, not inflated.

What is the difference between filler and a biostimulator?

A hyaluronic acid filler adds precise, immediate volume and can be dissolved if needed. A biostimulator prompts your own collagen to build gradually over weeks, for a softer, broader restoration that lasts longer. Which one fits depends on whether the loss is focused or diffuse.

Is fat transfer better than filler?

It is different, not simply better. Fat transfer uses your own fat for a lasting, one-time restoration and is a surgical procedure, so it suits people wanting a durable result. Filler and biostimulators are nonsurgical and adjustable. The right choice depends on how much volume is missing and how you feel about surgery.

Does sun exposure really affect facial volume?

It affects the collagen that supports the skin. Ultraviolet light breaks down dermal collagen faster than the body rebuilds it, thinning the support layer over years, which is one reason daily sun protection matters on the Central Coast.
